How to Apply
You can apply online at
ACCESS.wi.gov,
by mail, phone or in person with the agency.
If you choose to apply by mail, complete the BadgerCare Plus Application Packet (F-10182).

Your completed and signed and application, along with any required proof
(see Proof/Verification) should be sent to:
