Medical Home for Individuals Leaving
the Criminal Justice System
The Department will study the feasibility of submitting a state plan amendment to create a medical home for individuals who are exiting the criminal justice system. The medical home will coordinate the
individual’s care to ensure cost effectiveness while improving the overall quality of care.
