Our Philosophy and Goal

It is the desire of the Department that CRS consumers have many excellent service providers in their area to choose from as they work toward their recovery. To this end, we have created this webpage to allow qualified providers to express their interest in becoming a CRS provider. Upon receipt of this information, we will forward it to the county or tribal human services agency in your area and request that they contact you to follow up. Our goal is to create an environment in which consumers have a choice in their service provider; and service providers have fair access to Medicaid consumers in their area.

Providers Express Interest

Qualified providers can contact the county or tribal human services agency in your area to express interest in becoming a CRS service provider. Providers can also contact the CRS Coordinator at the State Department of Health Services, Division of Mental Health and Substance Abuse Services, Bureau of Prevention, Treatment and Recovery at michaelb.jones@wisconsin.gov. The state staff will forward your information to the appropriate county/tribe staff. Ultimately, the CRS contractual relationship will be between the service provider and the county/tribe human services agency.

Services Reimbursed through CRS

We've posted the CRS service definitions for Community Living Supportive Services, Peer Support Services, and Supported Employment on the web for you to review. If you provide one or more of these services and are not currently contracted with your local county or tribal human services agency, this may represent an opportunity for you to make these connections. Your reimbursement for provision of CRS services will come through the county or tribal human services agency under your contract with them.

CRS Service Provider Requirements
The Medicaid-certified CRS county or tribal provider retains all legal and fiscal responsibility for the services provided by their service contractors. Contracted CRS providers do not need to be individually certified by Wisconsin Medicaid. The Medicaid-certified CRS county or tribal provider is responsible for ensuring that its contractors meet all provider qualifications as outlined in the CRS State Plan Amendment (SPA) and the ForwardHealth Update issued October 2010. These qualifications include license and/or certification requirements and other applicable standards.

Each contracted CRS service provider is required to complete the CRS Benefit Provider Agreement and Acknowledgement of Terms and Participation form. There are two types of agreement forms. The DHS Form F-00312 is for provider entities who are corporations or legal businesses. The DHS Form F-00312A is for individuals or non-specified community providers. The Medicaid-certified CRS county or tribal provider shall execute on behalf of the Department the appropriate agreement with all CRS service providers. The Medicaid-certified CRS county or tribal provider shall retain such forms and renew them annually as required. All Medicaid-certified CRS county or tribal providers and contracted providers are required to follow all policies and procedures in the Online Handbook.
