Great Rivers Consortium to Manage Health Care, FoodShare, Caretaker
Supplement Programs starting January 3, 2012
MADISON—Beginning January 3, 2012, individuals in Barron, Burnett,
Chippewa, Douglas, Dunn, Eau Claire, Pierce, Polk, St. Croix, and
Washburn counties who need assistance with FoodShare, BadgerCare
Plus, Medicaid, Family Planning Only Services or Caretaker Supplement
should contact the Great Rivers Consortium to apply for benefits, get
information about benefits, and report changes:

Individuals seeking in-person assistance should call the Great Rivers
Consortium for the nearest location.
In order to improve customer service and manage public assistance
programs in a more cost-effective way, the Department of Health Services
has worked with counties and agencies to form groups called consortiums.
There are a total of ten consortia.
This change does not affect enrollment or the benefits members
receive. Access to services will remain at the same level. For more
