DEPARTMENT HOLDS INFORMATION SESSIONS FOR FAMILY CARE COMMUNITY HEALTH
PARTNERSHIP MEMBERS AND PROVIDERS
MADISON—The Department of Health Services in collaboration with local
partners is inviting members and providers of Community Health
Partnership to attend an information session regarding the transition to
a new managed care organization. Members will receive written notice of
the meetings as well.
During the sessions members and providers will hear about the program
choices available and will have an opportunity to hear from and meet
with the new agency that will be providing Family Care in the area.
There will also be representatives from Aging and Disability Resource
Centers and Ombudsman that can also answer questions.
“In an effort to ensure a smooth transition, we are hosting
informational sessions that members and their families and guardians, as
well as health care providers can attend,” said Deputy Secretary Kitty
Rhoades. “We encourage people to select the location and time that is
most convenient for them, however, members do not need to attend a
meeting in order to enroll into a new program.”
