This page provides online access to reference materials for county officials, individuals responsible for preparing and/or filing vital records, and those using vital records in family history or other research endeavors.

For fraud control purposes, detailed instructions for completing, filing and amending vital records are not included here.

  • Wisconsin County Cremation/Disinterment Permits and Death Certificate Process Fees  (PDF, 18 KB) (April 2011)  
    The County Board of each county sets fees for cremation release, disinterment permit and Coroner/Medical Examiner case filing.  This list shows the fees for these services by county.  Fees are subject to change.  Always check with the appropriate Coroner or Medical Examiner about the current fees in a particular county.

     The County Board of each county sets marriage license fees.  This list shows the fees for obtaining a license to marry in the 72 county offices in the state.  Fees are subject to change.  Always check with the appropriate County Clerk about the current fees in a particular county.

  • Military Time Conversion Chart
    This chart shows time in both regular (12-hour, AM PM) and 24-hour military time.  When possible, military time (24-hour clock) should be used when entering times on vital records.

  • Guidelines for Reporting Occupation and Industry on Death Certificates (PDF, 3673 KB; exit DHS) is a National Center for Health Statistics publication that provides guidelines to funeral directors for reporting industry and occupation on death certificates. (The Wisconsin Vital Records Office no longer supplies hard copies of this document.)
