Foster Care Medical Home (FCMH)
The State of Wisconsin will be implementing a new
health care program for children and youth in foster care and other
out-of-home care that will improve the quality, timeliness, and access of
health services for children and youth in out-of-home care. The general
intent of the foster care medical home (FCMH) program is to offer
comprehensive, coordinated services for children in foster care in
Wisconsin. A medical home in this context means that there is effective care
coordination across multiple disciplines within the health care system and
with external providers; it does not imply a specific provider location.
Specific goals of the FCMH program include:
- Integrated and Comprehensive Health Service Delivery. The FCMH
program will deliver coordinated, comprehensive health care including
physical, behavioral and oral health care that is tailored to each
child’s individualized needs.
- Timely Access. The FCMH program will provide timely access to a full
range of developmentally appropriate services. The needs of the
individual child will be assessed by an initial health screening within
48 hours of entering out-of-home care, followed by a comprehensive
health assessment within 30 days of enrollment. Children will receive
well child check-ups at the increased frequency for foster children
recommended by the American Academy of Pediatrics. All other medical,
behavioral, mental health, and oral health needs of the child will be
met in an effective and timely manner.
- High Quality and Flexibility of Care. The FCMH program will
coordinate, organize, and facilitate care in order to deliver services
in an effective and efficient manner. The FCMH will be expected to
utilize trauma-informed and evidence-informed practices. The FCMH will
have the flexibility to deliver services in the most effective manner,
including in home settings.
- Transitional Planning and Cross-System Coordination. Children in
out-of-home care will receive the transitional planning and follow-up
services necessary to assure continuity of health care after achieving
permanency or aging out of foster care. The FCMH program will coordinate
with other systems providing health and developmental services to the
child, including the local school system, the county-administered Birth
to 3 and Children’s Long Term Support programs, and county-funded mental
health services.
- Well-Being Outcomes. The FCMH program will support children to have
better health, improved behavior and mental health, positive permanency
outcomes, and enhanced resiliency.
