Application Process & Materials
If you are interested in applying to offer a Nurse Aide Training
Program either for your Wisconsin facility or as a private program, please contact the Office of Caregiver Quality at DHSCaregiverIntake@wisconsin.gov
to schedule an orientation appointment. All interested parties must
attend an orientation meeting regarding the Nurse Aide Training Program
application process and receive the
application materials.
