Lyme Disease: Health Professional Resources

Lyme disease has been officially reportable to the Wisconsin Division of Public Health (DPH) since 1980. The surveillance system is based on passive reporting of cases by laboratories, clinics and physicians. All patients with Lyme disease are to be reported to the local public health department in the jurisdiction where the patient resides; the reports are then forwarded to the Wisconsin DPH.
