How to help the Disability Determination Bureau process your claim

Be sure the Disability Determination Bureau (DDB) has your
current address and phone number. Any time your address or phone number
changes after you've filed your disability application, DDB needs to know.
Call 1-800-462-1938 to report these changes.

Provide complete names, addresses, and dates of treatment
for doctors, clinics, hospitals, and other health professionals who have
treated you for your medical conditions.

If you see a new healthcare provider, are hospitalized, or
develop new conditions, please let DDB know.
- You may be scheduled for a special examination by a doctor, which DDB
will pay for. If so, be sure to attend this examination. DDB needs
the findings from these examinations to determine the full extent of your
condition and how it affects you.
- Complete and return any additional forms that DDB sends to you as soon
as possible. Additional information is often requested about your work or
your daily activities so that DDB can better understand your condition.
