How to Electronically Order Diabetes Program Forms and Publications

All Wisconsin Diabetes Prevention and Control Program publications are available for download at no charge.  All publications are copyright free and you are welcome to add your corporate logo prior to printing.

A select number of publications are available to order in printed format.  Please click on the “Publication Request Form” link below, insert your information and desired quantities, save to your computer and email as instructed.  Publications available in printed format include:

• Diabetes Self-Care Information and Record Booklet (English, Spanish)
• Personal Diabetes Care Record (Wallet Card) (English, Spanish)
• Blood Sugar Log Booklet (English, Spanish)

To order printed materials:

1. Open Publication request form. (Word, 162 KB)

2. Fill in the shipping address (use street address only, no post office boxes), date, telephone number and email address.

3. Request only the quantity you will use over the next 3-6 months.

4. Save the completed form to your computer and email to the DPH Forms Manager.
