Living Well with Chronic Conditions

Overview

Living Well with Chronic Conditions (the Chronic Disease Self-Management Program) is a 6-week workshop that provides tools for living a healthy life with chronic health conditions, including diabetes, arthritis, asthma and heart disease.  Through weekly sessions, the workshop provides support for continuing normal daily activities and dealing with the emotions that chronic conditions may bring about.

Living Well helps people with chronic conditions make the most of their medical care and communicate more effectively with their healthcare providers.  The workshop does not conflict with ongoing programs or medical treatment.  It is designed to enhance regular treatment and disease-specific education such as cardiac rehabilitation, better breathing, or diabetes instruction.  In addition, many people have more than one chronic condition.  The program is especially helpful for these people because it teaches skills that help them manage their health and stay active.  Research has shown that up to two years after taking the workshop series, compared to the control group, participants reported fewer medical visits and self efficacy remains improved.

Objectives

  • Promote self-management of chronic conditions through the CDSMP by building and enhancing the existing infrastructure
  • Develop and provide culturally competent CDSMP for Hispanic and Native American populations
  • Implement Stepping On (falls prevention) in at least one planning service area (Northeastern Wisconsin)

Target Population

The target population for this grant is adults 60 years and older with specific outreach to Hispanics and Native Americans.  However, the program is open to people of all ages.

Anticipated Outcomes

  • Improved health status including reductions in disability, social/role limitations, pain and physical discomfort and an increase in energy
  • Improved self-management behaviors
  • Reduction in falls, falls-related emergency room visits and hospitalizations and improved fall self-efficacy

The Wisconsin Department of Health Services supports Living Well/CDSMP workshops and Stepping On/Falls Prevention classes by providing opportunities for leader training, coordination, and networking, coordinated through Area Agencies on Aging.  The workshops/classes are held at various times and locations in Wisconsin throughout the year.
