Referrals:
Made by county case manager or Managed Care Organization (MCO)
Pre-admission assessment:
ITP team member meets in individual’s community setting
Team identifies current concerns and determines primary focus of potential treatment plan
Admissions committee reviews assessment
Admissions coordinator notifies referring party of decision
Date of admission/discharge are set
Day of Admission:
Tour of facility and introductions to inter-disciplinary team
Admission staffing:
Review care plan
Summarize treatment goals
Decide projected discharge date
Complete medical history and physical exam
Discharge:
Findings and recommendations reviewed with parents/guardians and
community caregivers at the discharge staffing
Social worker/program coordinator available for follow-up or
questions/concerns
Training offered for parents/guardians and community support staff
Documentation:
Written recommendations will be mailed within two weeks of discharge
