Your agency has been identified for receiving Communicable Disease Control and Prevention funding. As noted in your scope of work, as well as in Wisconsin State Statute, one of the requirements for this funding is submitting an annual report. This report should identify how the funds were allocated during the designated funding period to address communicable disease control and prevention efforts. We utilize your responses to help advocate for future funding to support local and tribal health agencies for communicable disease control and prevention efforts.
