Medicaid for the Elderly, Blind or Disabled

Medicaid provides health care coverage to people who are age 65 and older, blind, or who have a disability. Disability and blindness determinations are made by the Disability Determination Bureau (DDB) in the Department of Health Services.

How to Apply

You can apply for all plans listed below by mail, phone, in person or online at ACCESS.wi.gov.

  • Institutional Medicaid (Hospital, Nursing Home, Institutions for Mental Disease)

  • Medicaid Standard

  • Medicaid Purchase Plan (MAPP)

  • Emergency Services

  • Tuberculosis-Related Services Only Benefit

  • Medicare Savings Program

To apply for Long-Term Care services through Home and Community Based Waivers, Family Care, Partnership or PACE you must contact your local Aging and Disability Resource Center (ARDC). To get the address and phone number of your ARDC, go to dhs.wisconsin.gov/LTCare/adrc/ or call 1-800-362-3002. If your county does not have an ADRC, contact your local Social or Human Services Department for information on requesting these services.

If you choose to apply by mail, fill out the Medicaid for the Elderly, Blind or Disabled Application Packet (F-10101). You can get the application from your agency, by calling Member Services at 1-800-362-3002 or online at dhs.wi.gov/em/customerhelp.

You will be notified about your enrollment, in writing, within 30 days from the day the agency gets your application.
