  1. (Q) Who to contact for Food Manager Certification courses available and the requirements?

    (A) A complete list of approved courses can be received by contacting the Food Safety and Recreational Licensing Section of the Division of Public Health. They can be reached at 608-266-2835 by mail at:  P O Box 2659, Madison, WI 53701-2659 or by email at Katherine.Braumann@wisconsin.gov. Some of the courses that are approved are the Wisconsin Restaurant Association (WRA), Tavern League of Wisconsin and most of the Technical Schools in Wisconsin.

  2. (Q) How do I to get a duplicate copy of a Food Manager Certificate?

    (A) You need to submit your request for a duplicate certificate in writing. Please include your name, address, social security number, and a check or money order for $10.00 made out to the Division of Public Health. Mail the request and fee to:  P O Box 2659, Madison WI 53701-2659.  (Please allow 2 weeks for processing.)

  3. (Q) Can I get reciprocity for my food manager certification exam that I took in (another state name) and not have to retake the exam in Wisconsin.

  4. (A) If you took and passed a national exam (such as the Serv-Safe exam) that is accepted in Wisconsin, and took it within three years of application for certification in Wisconsin, we would accept it and not require you to retake an exam here.

  5. (Q) How do I get recertified (or a renewal) for Food Manager Certification in Wisconsin? (Do I have to take a course and or a test to do so?)

    (A) For recertification you need to take an approved recertification course, about a 4-hour course, or retake and pass the original certification exam. Some of the courses that are approved are the Wisconsin Restaurant Association (WRA), Tavern League of Wisconsin and most of the Technical Schools in Wisconsin.  A complete list of approved courses can be received from our office by phone at 608-266-2835 by mail at:  P O Box 2659, Madison, WI 53701-2659 or by email at Katherine.Braumann@wisconsin.gov. Once you have done that you need to submit proof of having taken the recertification course or that you have retaken and passed the exam along with either a renewal form and $10, or an application and $10.  Your certificate will then be renewed for 5 years.

     

  6. (Q) Who do I call to find out if someone is certified in Wisconsin as A Food Manager?

    (A) To receive an answer to this question, you need to contact the Food Safety and Recreational Licensing Section of the Division of Public Health.  They can be reached at 608-266-2835. 

  7. (Q) How do I verify my certification as a food manager in Wisconsin?

    (A) To find out if you are currently certified in the State of Wisconsin you need to contact our office at 608-266-2835.  If you give them your name and social security number they will be able to check to see if you are currently state certified. 

  8. (Q) Who do I contact to get an inspection done on my facility so that I can have my permit issued to start operating my business?

    (A) To receive an answer to this question, you need to contact the Food Safety and Recreational Licensing Section of the Division of Public Health.  They can be reached at 608-266-2835.  Based on your city and county location they will be able to give you the name of either the Field Inspector or the Agent Health Department and their phone number for you to contact to get inspected and licensed by.

  9. (Q) Who do I speak to about getting a permit/license for a facility?

    (A) Licensing is done by Food Safety and Recreational Licensing Section of the Division of Public Health.  They can be reached at 608-266-2835 by mail at:  P O Box 2659, Madison, WI 53701-2659 or by email at DHSFSRLGroup@wisconsin.gov.

  10. (Q) What agency does the issuing of licenses/permits for bakeries?

    (A) Bakeries are licensed by the Department of Agriculture, Trade and Consumer Protection, Division of Food Safety, 2811 Agriculture Drive, Madison, WI  53704-6777. The  phone number is 608-224-4700.

  11. (Q) How do I get a duplicate copy of my license/permit for my business?

    (A)  You need to submit your request for a duplicate permit in writing.  Please include your name, address, the facility name, the facility address, and a check or money order for $10.00 made out to the Division of Public Health.  Mail the request and fee to:  P O Box 2659, Madison WI 53701-2659.  (Please allow 2 weeks for processing.)

  12. (Q) When do my licenses/permits expire?

    (A) All licenses expire June 30th of each year.  Licenses are not prorated.  If a new license is issued after April 1st it does not expire until June 30th of the next year (example if a license is issued 4/10/04 it will expire 6/30/05).  Licensing is done by Food Safety and Recreational Licensing Section of the Division of Public Health.  They can be reached at 608-266-2835 by mail at:  P O Box 2659, Madison, WI 53701-2659 or by DHSFSRLGroup@wisconsin.gov if you have further questions.

  13. (Q) How much does it cost for a license/permit for operating my business?

    (A) To receive an answer to this question, you need to contact the Food Safety and Recreational Licensing Section of the Division of Public Health.  They can be reached at 608-266-2835. There are various questions that will need to be asked of you before your question can be answered. Such as type of facility, where located, etc.

  14. (Q) I received a renewal but my business is Out of Business, what do I have to do?

    (A)  The first question on the renewal form asked if you are operating. Check no and send it back to us so that we can change our records.

  15. (Q) I have received more then one renewal for my various licenses/permits can I write one check to cover all the renewals?

    (A)  Yes, you can write one check, but be sure to write the ID NUMBERS for each permit on the check and include the renewal form for each permit you are paying for.
