Prenatal Care Coordination

Prenatal Care Coordination (PNCC) is a Medicaid and BadgerCare Plus benefit that helps pregnant women get the support and services they need to have a healthy baby. PNCC services include:

  • Help getting the health care you need
  • Personal support
  • Information on good eating habits and health practices
  • Help finding needed services in your community

The purpose of the Medicaid Prenatal Care Coordination program is to provide access to medical, social, educational, and other services to pregnant women who are considered high risk for adverse pregnancy outcomes. The components of this benefit are outreach, assessment, care plan development, ongoing care coordination and monitoring, and health education and nutrition counseling. Some key outcome indicators include: Tobacco Exposure, Alcohol Use, Breastfeeding, Safe Infant Sleep Practices, Perinatal Depression, Family Planning, and Involved Father.

2012 Data Sheets

The following data represent selected outcomes for women statewide and by region who received Medicaid PNCC. Data are from the Maternal and Child Health Reporting System, part of the Secure Public Health Electronic Record Environment (SPHERE), and are reported annually.

Data Sheet Instructions P-00549I (PDF, 189 KB) - Each regional data sheet is a fillable Word file, so you can customize the data sheet for your region to include data for your health department or other locality. (You can also fill in your locality’s name in the data sheet title.) To fill in the “local” data column in your data sheet, follow the Data Sheet Instructions.
