Transition to New Managed Care Organization in Eau Claire, Chippewa, Dunn, Pierce and St. Croix Counties

Information for former Family Care Community Health Partnership (CHP) Members and Providers

On January 1, 2013, the Department of Health Services contracted with a new managed care organization (MCO) to provide Family Care services in Eau Claire, Chippewa, Dunn, Pierce and St. Croix Counties. 

Southwest Family Care Alliance (exit DHS) is the new MCO for this region. If members, guardians, or providers have questions about this transition, contact Southwest Family Care Alliance at 1-888-316-5306 or expansion@familycarealliance.org.
