Healthy Birth Outcome Medical Home for Pregnant Women
The Department will submit a state plan amendment to create a medical home for pregnant women on fee-for-service Medicaid. The medical home will coordinate care for the pregnant mother and her baby to ensure a healthy pregnancy.
