PROVING CITIZENSHIP AND IDENTITY FOR WISCONSIN MEDICAID/BADGERCARE/FAMILY PLANNING WAIVER PROGRAM

Federal law requires that all persons applying for, or getting Medicaid, BadgerCare or Family Planning Waiver Program benefits who say they are a U.S. citizen must show proof that they are a U.S. citizen and proof of their identity (who you are). If you are applying for benefits, you will have at least 30 days to provide proof to the Medicaid office. If you are already getting benefits, you will need to give proof to your worker at your next review.

If you have provided this information in the past, it may already be on file; your worker will let you know if s/he needs more proof.

This law does not affect you if you are a qualified alien, Medicare or SSI recipient.

You can use one item from the list below.

To prove citizenship and identity, you can use:

  • U.S. Passort
  • Certificate of U.S. Citizenship
  • Certificate of U.S. Naturalization

Or, you can use one item from each list below.

To prove you are a citizen, you can use:

  • U.S. Birth certificate
  • U.S. State Department Report of Birth Abroad
  • U.S. Citizenship ID Card
  • Adoption papers showing U.S. birth
  • U.S. Military Record of Service
  • Hospital record of U.S. birth
  • Life or health insurance record showing U.S. birth
  • Nursing home admission papers showing U.S. birth

and

To prove who you are, you can use:

  • State driver license
  • ID card issued by federal, state or local government
  • School ID with picture
  • U.S. Military ID card or draft record
  • U.S. Military dependent ID card
  • Native American Tribal document
  • For children under 16, a signed Statement of Identity form (HCF 10154)

You may bring original or photocopies of these items to your worker. If you are doing your application by mail or online, send photocopies of these items. If you are having trouble getting what you need, contact your worker for help.
