What is HealthCheck?
HealthCheck (Wisconsin’s EPSDT Program) is a preventive health check-up program for anyone under the age of 21
who is currently eligible for Wisconsin Medicaid or BadgerCare Plus.
Why should my child or I get a HealthCheck
Exam?
HealthCheck teaches you and your child how to prevent illness, and can also find health problems early, before they become serious.
(Remember a healthy child is a happy child!) HealthCheck meets the physical exam requirements for programs such as Head Start, Child Care or WIC (Women, Infants and Children Supplemental Food Program) and school physicals.
A HealthCheck exam may also make you eligible for some services not normally paid for.
What if a health problem is found?
HealthCheck will help you make an appointment for any follow-up care or treatment.
HealthCheck can also help you find a doctor or dentist if you need one.
What about transportation?
If you need transportation to your appointment, please contact
LogistiCare directly at (866) 907-1493 (Voice) or (866) 288-3133 (TTY).
