CRS County/Tribal Application Frequently Asked Questions (FAQ)
Is there a date that you need our certification application by? A due date has not been specified. The county however, must have the application in and correct before it can bill for services. Do I need to submit the county/tribal agency application at the same time as the Individual Service Plans? Or can I submit the Application first? You must submit the county/tribal agency application prior to the individual participant packets. Once your county or tribe has been approved as a CRS provider, you may begin submitting consumer service plans. How does the State determine the start date for reimbursement purposes? The start date for reimbursement purposes will be the later of the date the State receives your "approvable" county/tribal application, or the effective date of the consumer's service plan, as specified in the CRS approval letter. Do I need to submit copies of job descriptions for the county/tribal employees who will be performing assessments for CRS consumers along with our CRS application? Yes, the Department reviews the job descriptions submitted with the CRS application to gain assurance that the staff who will be performing assessments of CRS clients have the proper qualifications. Do I need to submit a separate listing of vendors with which my county or tribe will contract with to provide CRS services, along with our CRS application? Counties and tribes are welcome to provide a separate list of vendors, or to incorporate this list into the application itself in section VII A “Provider Network." Either way, counties and tribes must show that they have a network of providers that is capable of providing all three of the services called for under the CRS benefit.
