Voluntary and Involuntary DischargeReturn to the Community Recovery Services Home Page The Consumer Discharge ProcessDischarging your CRS consumer is an active process. The Department's expectation is that care managers will inform the CRS Team as soon as they are certain that a discharge is to take place in order that the consumer's records may be properly updated. The Department has prepared the guidance below to assist care managers as they navigate the discharge process. Please review this information to determine your responsibility for notifying the Department. DischargeDischarge from CRS shall be based on the discharge criteria in the service plan of the consumer unless any one of the following applies:
When a consumer is discharged from the CRS program, the consumer shall be given written notice of the discharge. The notice shall include all of the following:
Note: A written request for review of the determination of need for psychosocial rehabilitation services should be addressed to the Bureau of Prevention, Treatment and Recovery, 1 W. Discharge SummaryThe county/tribal Medicaid agency shall develop a written discharge summary for each consumer discharged from psychosocial rehabilitation services.
