Welcome to the 2009 Family Care Partnership contract between Wisconsin's Department of Health Services, Division of Long Term Care and the Partnership Managed Care Organizations. This generic contract is intended for local county/tribal agency Income Maintenance workers, workers at ADRCs, advocates and others who determine eligibility A person is eligible for membership in the Partnership program if the person meets all eligibility requirements defined in the 2009 contract. and issue benefits for Family Care Partnership.  

Family Care Partnership is an integrated health and long-term care services for eligible members.
