License Annual Fees
A general licensee shall
pay an annual registration fee of $100 per site and $50 per device.
The department may not
assess an additional fee for recording changes in registration
information.
The annual registration fee for the next year shall be
paid by December 31 of the prior year of registration. The department
shall issue a notice of registration following receipt of the registration
fee. If the annual registration fee for the next year is not received by
the department by December 31 of the prior year of registration, a license
shall pay a penalty fee of $25, in addition to the registration fee and
regardless of the number of devices, before the department will issue a
new notice of registration.
