Wisconsin Children and Youth with Special Health Care Needs (CYSHCN) Statewide InitiativesRFP # G1679-DPH-BCThe Wisconsin Department of Health Services, through the Division of Public Health, announces the release of approximately $235,000 in funding for 4 separate CYSHCN statewide initiatives:
Each CYSHCN Statewide Initiative will create a “hub of expertise” around the specified CYSHCN National Performance Outcome (NPO). Funded Initiatives will: coordinate state CYSHCN efforts around the National Performance Outcome; disseminate national information to Wisconsin stakeholders; and have the capacity and motivation to seek additional federal dollars to build on the National Performance Outcome area; All Initiatives will have a statewide focus, serving the entire state and providing representation on statewide councils and boards as appropriate. Funded CYSHCN Statewide Initiatives are expected to keep current on national work in these areas, present at relevant state and regional conferences, and promote the work being done in Wisconsin. This funding is available through annual contracts for the grant cycle period beginning January 1, 2011 through December 31, 2015.
