MDS 3.0 Section Q
Referral Process
Nursing homes that participate in the Medicare or Medicaid programs
must complete the Minimum Data Set (MDS) assessment for all residents
admitted to the facility. Nursing Homes are required to make a referral to
the designated Local Contact Agency (LCA) for any resident who indicates
they wish to talk to someone about returning to the community. The
Referral Management Module is a web-based application used to automate the
MDS 3.0 Section Q Referral Process. This website provides training modules
and information pertaining to the Referral Management Module.

Core Training Module
This training module will cover the steps necessary in order to receive
access to the Nursing Home Referral Management Module in the Program
Participation System (PPS). Please review this
module prior to the process specific modules below. LCA staff members who
have a WAMS ID and PPS access must gain access to the Referral Management
Module by following instruction is this training.
