The purpose of this memorandum is to provide information related to
revisions to the Guidelines for Care Coordination for Hospice Patients
Who Reside in Nursing Homes.
In 1995, the hospice and nursing home industries published the first
edition of the Guidelines for Care Coordination for Hospice Patients
Who Reside in Nursing Homes. The guidelines were distributed to all
hospice and nursing home providers via BQA memorandum in January 1996.
Through the years since publication of the first edition, a great deal
has been learned. In response, the Bureau of Quality Assurance (BQA) in
collaboration with the Hospice Organization and Palliative Experts (HOPE)
of Wisconsin, the Wisconsin Health Care Association (WHCA) and the
Wisconsin Association of Homes and Services for the Aging (WAHSA)
published the second edition of the guidelines. A copy of the revised
guideline is attached. This document may be reproduced as needed.
These guidelines are not regulation and are not meant to be a guarantee
of compliance with state and/or federal hospice or nursing home
requirements. They have been developed to provide a source of information
to assist and enhance the partnership between hospice and nursing home
providers, each with distinct roles in planning and providing the highest
quality services to a common patient during the final months and days of
their life. Providers that use these guidelines must develop provider
specific written policies and procedures to provide direction to staff for
their use.
