Bureau of Assisted Living Restructuring to Four (4) Regions
As you are aware, the State of Wisconsin is facing a challenging
financial future. As a result, the Division of Quality Assurance has made a
difficult decision related to regional operations within the Bureau of
Assisted Living.
Effective April 1, 2009, the Bureau of Assisted Living will cease
operations in the Northern Regional Office located in Rhinelander.
Facilities previously assigned to the Northern Regional Office will be
reassigned to the Western Regional Office in Eau Claire, the Northeastern
Regional Office in De Pere, and the Southern Regional Office in Madison.
Please see the attached map and Bureau of Assisted Living telephone
directory illustrating the changes in regional boundaries and Bureau of
Assisted Living staff contact information. Department websites will be
updated on or about April 1, 2009, reflecting these changes. If you have any
questions regarding these changes, please contact the Assisted Living
