Changes in the Division of Quality Assurance Bureau of Assisted Living - Southern Regional Office

The purpose of this memo is to inform providers of a change in location for the Division of Quality Assurance, Bureau of Assisted Living (BAL), Southern Regional Office.

BAL Southern Regional Office staff will be moving to a different floor/office within 1 West Wilson Street, Madison. Effective April 19, 2012, BAL Southern Regional Office staff will be located in Room 950. Effective April 19, 2012, all correspondence for the BAL, Southern Regional Office, should be mailed to the following addresses:

The Southern Regional Office main telephone number will continue to be (608) 264-9888, however, the Southern Regional Office fax number will be changing to (608) 264-9889.  All other BAL staff telephone numbers will remain the same.
