Changes in the Division of Quality Assurance Southern Regional Office - Bureau of Assisted LivingThe purpose of this memo is to inform providers of changes in the Division of Quality Assurance, Southern Regional Office, Bureau of Assisted Living (BAL). As you may be aware, Susan Blount, Assisted Living Regional Director, is retiring effective June 30, 2011. Mark Andrews has been reassigned effective May 5, 2011, as the Assisted Living Regional Director for the Southern Regional Office, Bureau of Assisted Living. In order to streamline our operations, and implement more efficient and effective practices, Southern Regional Office BAL staff currently housed at 2135 Rimrock Road, Madison, will be moving to 1 West Wilson Street, Room 1151, Madison, WI 53703. The move will take place during the week of June 20-24, 2011. All other BAL staff telephone numbers will remain the same.
