Long Term Care Baseline Prevention Practices Assessment Online Survey

The purpose of this memo is to announce the availability of the Long Term Care Baseline Prevention Practices Assessment Online Survey located at http://4.selectsurvey.net/DHS/TakeSurvey.aspx?SurveyID=l6009ol3

The Wisconsin Healthcare-Associated Infections (HAIs) in Long-Term Care (LTC) Coalition Steering Committee developed this online prevention practices assessment survey based on the CDC LTC Baseline Prevention Practices Assessment Tool to evaluate the status of infection prevention and control efforts in Wisconsin LTC facilities. Please refer to DQA Memo 12-016, Wisconsin Healthcare-Associated Infections (HAIs) in Long-Term Care Coalition.

While completion of the survey is voluntary, the Wisconsin HAI in LTC Coalition requests that each LTC facility in Wisconsin, with input from the facility's infection preventionist, complete the anonymous online survey once by November 30, 2012. Survey results will be used to assist the Coalition in planning education and training on prevention and treatment of HAIs.

The aggregate results of the survey will be shared with the CDC as well as Wisconsin's LTC community. The survey will be repeated in two years to measure practice change(s) over time.
