If you need help caring for your child with long-term support needs, the Children's Community Options Program (CCOP) may be able to provide funding and service coordination for your family. CCOP funding can be used for a range of supports and services to help you care for your child at home. Supports may also help your child take part in family and community activities.
A local service coordinator will work with your family to establish eligibility, assess your child, and create a service plan. The assessment is a chance to identify your and your child’s unique circumstances, strengths, needs, and outcome goals. Together, you and the service coordinator will explore different supports and services to help your child and family reach your desired outcomes.
