2024 FoodShare Program Customer Service Survey

Each year, the Wisconsin Department of Health Services (DHS) surveys FoodShare applicants and members about the customer service their agency provided when processing their FoodShare application, conducting their FoodShare interview, and answering questions about their case. The information you provide in this survey will be used to improve customer service at the local office where you get services. Please do not list your name, address, or phone number on the survey. Your responses will be kept private and will not affect your benefits in any way.
