Dane County Waiting List Management Policy While Transitioning to Family Care and IRIS

This document defines the state requirements for the management of a waiting list of qualified applicants for the Medicaid adult long-term care programs of Family Care, Partnership and IRIS (Include, Respect, I Self-Direct) when a county transitions from adult legacy waiver programs to Family Care, Partnership and IRIS. In Dane County, Partnership with a limited benefit package is at entitlement for individuals that meet the target groups for physical disability or frail elderly.
