If you do not renew your Wisconsin EMS license by June 30, 2018, you may not function as an EMS provider after that date.
According to DHS 110.05 (2), a person acting or advertising as an EMT or first responder without a license or certificate issued by the department or without appropriate credentialing may be subject to department action under sub ch. V or s. 256.15 (11) (c), (d), or (f), Stats., as applicable.
Starting July 1, 2018, the 2018-2020 EMS Provider Late Renewal Application will be available and accessible until December 31, 2018. To be considered, the completed application must be submitted to E-Licensing prior to this date. In addition, under DHS 110.16 (1) (a), the applicant for a late renewal will be charged a $50.00 late renewal fee.
The Wisconsin EMS Office issues only a single license at your highest eligible level (your "primary license level") based on your education and training. Complete only one (1) 2018-2020 EMS Provider Late Renewal Application.
Local credentialing is a process for obtaining both service director and medical director authorization to practice with a specific service and is done electronically using the "Local Credential Agreement" (LCA) form through your E-Licensing account. If your license has not been renewed, you are NO LONGER listed as being on a service's roster (see your E-Licensing profile), and you must complete a LCA for that service in order to operate for them with your license. The LCA process is required for each new service you join.

Log into your E-Licensing account by entering your user name and password (see link at the beginning and end of these instructions).
- If you do not have your user name and password you may attempt to recover your logon information by going to the E-Licensing logon page and clicking "Forgot Password." If your E-Licensing account has a valid email address on file, your logon information will be sent to you by email. If we do not have an active email address on file, contact the EMS office for assistance or send your name, license number, and current email address to: email@example.com for faster service.
- You may be required to change your password when you log in. If you see "Your password is expired" you have successfully logged into E-Licensing. Follow the directions provided to change your password to one you will remember using the required format.
- DO NOT share your password with anyone as your E-Licensing account contains personally identifiable information!
Once you have logged into E-Licensing and changed your password, use your user name and new password to log in; again, DO NOT share your password with anyone!
If you get an "invalid logon" message, try logging in using your WARDS user name and password, if known. If that does not work, try your E-Licensing user name and password again which may then 'sync' your accounts. Logons are case-sensitive so make certain you are entering your user name and password EXACTLY as provided using correct capitalization and no extra spaces. If you still get an error message, contact the EMS office at firstname.lastname@example.org for assistance.
Update Your E-Licensing Information
Once logged into E-Licensing, verify and/or update your profile with your current demographic information. We strongly recommend that you use a stable email address that will not change frequently. It is not necessary to add CPR and/or ACLS on your profile as it will be required on the renewal application. In your profile, select "Click here to show your current services" to make certain you are credentialed with the correct services. If you work with services not shown, you must complete a Local Credential Agreement (LCA) available through your E-Licensing account to be added to the service roster. You may not function as an EMS Provider if you are not properly credentialed with the service. If you are no longer on services showing in your list, contact the service director and ask that he/she remove you from their WARDS roster.
Have Your Renewal Information Handy
You will need the following information to complete your 2018-2020 EMS Provider Late Renewal Application:
- Initial or refresher EMS training completed between July 1, 2016 and December 31, 2018.
- Your current CPR card.
- If you are renewing an Intermediate or Paramedic license, your current ACLS card.
- If you received a felony or misdemeanor conviction since your last successful application process, have pending charges at this time or received a traffic violation that led to the suspension, revocation or withdrawal of your driver's license, all required court documentation. (You will have the opportunity to upload the documents if you have them electronically.)
- To begin the license renewal process, click on "Applications" the "2018-2020 EMS Provider Late Renewal Application ".
- If you do not see the renewal application, check your profile to make certain you are currently licensed with a June 30, 2018 expiration date.
- You may only renew at the level of your current license. If you are eligible to upgrade your license level or wish to downgrade your license level, you must complete the Provider Upgrade application or the Provider Downgrade application, NOT the 2018-2020 EMS Provider Late Renewal Application.
- Under "Form Pending Completion" you will find the "Renewal Part 1: Emergency Medical Services Workforce Survey." Click on "Start." Completion of this survey is required to renew your license. It is necessary to gather information about the EMS workforce to determine future EMS needs. Your answers are confidential and will not be released. Aggregate results will be posted on our website.
- Once you complete and submit the survey, the "2018-2020 EMS Provider Late Renewal Application" will be available under "Form Pending Completion." Complete the late renewal application answering all required questions. You may scan and upload relevant documents (such as court documents) to your application, if needed.
- Your application is not complete until it is signed electronically, using your E-Licensing user name and password, and all additional supporting documentation is received in the EMS Office. Do not submit copies of your refresher training (this information is posted to your E-Licensing account from your training center) or CPR/ACLS cards, but do keep them on file for at least five (5) years.
Any 2018-2020 EMS Provider Late Renewal Application not submitted to E-Licensing prior to December 31, 2018 will be considered a Reinstatement under DHS 110.09 (2).
Starting January 1, 2017, all 2014-2016 EMS Provider Licenses not renewed will be in Reinstatement status [DHS 110.09 (2)]. As of January 1, 2017, you will need to fulfill the following to regain your certification or license:
- Complete a Provider Reinstatement form in your E-Licensing account.
- Provide evidence of continuing education for the previous renewal cycle (2014-2016).
- $75.00 reinstatement fee.
- Complete an assessment exam (written and practical) through the National Registry of EMTs. Each person must have an approval letter from the Wisconsin EMS Office prior taking the exam.
- Proof of successful completion of the NREMT assessment exam (written and practical).
THE REINSTATEMENT PERIOD UNDER DHS 110 WILL BEGIN ON JANUARY 1, 2017 AND END ON JUNE 30, 2018
If you have not reinstated your license by June 30, 2018, your license will lapse. You will be required to complete an initial course of training to obtain a certificate or license (DHS 110.09).

The EMS Office has been conducting random audits of the renewal process. If you are selected for an audit, you must provide all requested documentation. Copies of all information entered on this 2016-2018 EMS Provider Late Renewal Application must be retained for at least 5 years. Failure to provide required documentation will result in the suspension or revocation of your EMS Provider license.
