Base Contract, Signature Pages and Addenda

Local health departments (LHD) may find their 2015 Consolidated Contracts and associated documents by following each of the four steps below. Please complete each step to ensure that you receive all of the necessary documentation pertaining to your 2015 DPH Consolidated Contract.

  1. Template MOU for Transferring Funds to a Consortium Fiscal Agent (Word,  21 KB)
    If an LHD has chosen to transfer funds to another LHD via a Memorandum of Understanding (MOU), both sending and receiving LHDs must sign and return the Template MOU along with the signed 2015 Contract Signature Pages. If multiple LHDs are transferring funds to one LHD, then there must be an MOU included with the Contract Signature Pages for each LHD that is transferring funds.

  2. 2015 DPH Base Contract (PDF,  116 KB)
    The Base Contract serves as the legal contract and describes the grantee's accountability for the program deliverables and funding provided in the 2015 Consolidated Contract Addendum.

  3. 2015 Base Contract Signature Page (Word,  124 KB)
    The Contract Signature Page includes a brief cover page, the DPH Base Contract Signature Page, the Anti-Lobbying Signature Page, and the Certification Regarding Debarment and Suspension Signature Page. In order to receive a January 2015 prepayment, two signed originals of each of these pages must be returned to the following address prior to November 28, 2014:

  4. Contract Addenda - 2015 Consolidated Contract Overview (Excel,  695 KB)
    To download your 2015 Consolidated Contract Addendum, open the 2015 Consolidated Contract Overview spreadsheet and click the link for your agency-specific addendum in the Base Contract Addendum column of the Overview tab. By way of reference, the addendum is considered an attachment to the Basic Contract and includes all programmatic objectives, quality criteria, and statements of work as well as the associated funding amount for each of the programs initially included in your 2015 DPH Consolidated Contract. Contract addenda are only available for LHDs that have completed negotiations in GAC for all of their allocated funding. The remaining addenda will be made available on this spreadsheet as the negotiations are completed and objectives are signed off on.
