Who do I contact for Food Manager Certification courses available and the requirements?

A list of course providers can be received by contacting the Food Safety and Recreational Licensing Section of the Division of Public Health. We can be reached by email at DHSFSRLgroup@wisconsin.gov, by phone at 608-266-2835, or by mail:  P O Box 2659, Madison, WI 53701-2659. Some of the course providers are the Wisconsin Restaurant Association (WRA), Tavern League of Wisconsin and many of the technical schools in Wisconsin.

How do I to get a duplicate copy of a Food Manager Certificate?

By email: There is no charge if we can provide your duplicate by email.  Contact us at DHSFSRLgroup@wisconsin.gov with your request and provide your full name, last four digits of your social security number, and your email address.  Or call us at 608-266-2835.

By mail: If you need the duplicate mailed to you, there is a $10 charge to cover postage and handling. Your request needs to be submitted in writing. Please include your name, address, social security number, and a check or money order for $10.00 made out to the Dept of Health Services.  Mail the request and fee to:  DHS-FSRL, P O Box 2659, Madison WI 53701-2659.  (Please allow 2 weeks for processing.)

Can I get reciprocity for my food manager certification exam that I took in another state so I do not have to retake the exam in Wisconsin?

Please email us at DHSFSRLgroup@wisconsin.gov or call 608-266-2835 to get accurate information regarding your specific situation.  Typically, as long as you took one of the exams approved for the Wisconsin Food Manager Certificate, and you have the certificate to verify you passed the exam, it does not matter where the exam was taken. The four approved exams are ServSafe, Food Safety Professionals, Prometric, and 360Training/Learn2Serve.  Exams must have been taken within the past 5 years and in a proctored setting.  A copy of the exam certificate, a $10 check or money order, and the completed application are required to receive the Wisconsin Food Manager Certificate.  However, there could be other restrictions that might prevent us from issuing a Wisconsin Food Manager Certificate, so it is best to call us.

How do I get re-certified (or a renewal) for Food Manager Certification in Wisconsin? (Do I have to take a course and or a test to do so?)

Recertification courses are no longer accepted after December 31, 2014.  As of January 1, 2015, all Wisconsin Food Manager Certificates require that you take and pass one of the approved food manager certification exams. The four approved national exams are ServSafe, Food Safety Professionals, Prometric, and 360Training/Learn2Serve.  All exams must be taken in a proctored setting.  A copy of the certificate verifying you have passed the exam, a check or money order for $10, and the application should be mailed to: DHS-FSRL, P O Box 2659, Madison, WI 53701-2659.  For additional information, contact them by email at DHSFSRLgroup@wisconsin.gov, or by calling 608-266-2835.

How do I verify if I have a current Wisconsin Food Manager certificate?

Who do I contact to get an inspection done on my facility so that I can have my permit issued to start operating my business?

Who do I speak to about getting a permit/license for a facility?

What agency does the issuing of licenses/permits for bakeries and coffee shops?

Most bakeries and coffee shops are licensed by the Department of Agriculture, Trade and Consumer Protection, Division of Food Safety, 2811 Agriculture Drive, Madison, WI  53704-6777. The  phone number is 608-224-4700.

When do my licenses/permits expire?

Licenses expire June 30th of each year.  Licenses are not prorated.  If a new license is issued after April 1st it does not expire until June 30th of the next year (example: if a license is issued 4/10/04 it will expire 6/30/05).  Licensing is done by the Food Safety and Recreational Licensing Section of the Division of Public Health.  We can be reached by email at DHSFSRLgroup@wisconsin.gov, by phone at 608-266-2835 by mail at:  P O Box 2659, Madison, WI 53701-2659.

How much does it cost for a license/permit for operating my business?

To receive an answer to this question, contact the Food Safety and Recreational Licensing Section of the Division of Public Health.  We can be reached by email at DHSFSRLgroup@wisconsin.gov, or by phone at 608-266-2835. There are various questions that will need to be asked of you before your question can be answered, such as type of facility, where it is located, etc.

I received a renewal but my business is Out of Business, what do I have to do?

The first question on the renewal form asks if you are out of business. Check yes and send it back to us so that we can change our records. 

I have received more then one renewal for my various licenses/permits. Can I write one check to cover all the renewals?

Yes, you can write one check. Be sure to write the ID NUMBERS for each permit on the check and include the renewal form for each permit you are paying for.
