All expire on June 30 of even-numbered years regardless of the issue date.

Prerequisites

Successful vehicle inspection by the Department of Transportation (DOT). For a copy of the requirements see Trans 309, Ambulance Inspections.

Application Process

Required Documentation

Operational plan, proof of medical malpractice insurance sufficient to protect the EMTs, and a roster of licensed EMTs and EMTs in training affiliated with the service.
