MDS 3.0 Section Q Referral Process

Nursing homes that participate in the Medicare or Medicaid programs must complete the Minimum Data Set (MDS) for all residents admitted to the facility. When a resident indicates that they would like to talk with someone about returning to the community, within ten (10) business days of completing Section Q, the nursing home is required to send the Nursing Home MDS 3.0 Section Q Referral, F-00311 (PDF) via the Automated Nursing Home MDS Section Q Referral Management System to the Aging and Disability Resource Center (ADRCs) serving the county where the nursing home is located. This website provides training modules and information on how to use the Referral Management System.

If the nursing home’s data system is not compatible with the Automated Nursing Home MDS Section Q Referral Management System, the nursing home should fax the Nursing Home MDS 3.0 Section Q Referral, F-00311 (PDF) to the designated ADRC.
