Vendor Resources - Submitting Complaints and Reporting Fraud and Abuse

The Wisconsin WIC Program takes all complaints and allegations of program violations, abuse and fraud seriously. Please report any issues immediately and in as much detail as possible. Complaints and reports may be made anonymously.

Submitting Complaints

To report a complaint about a WIC vendor or a WIC participant, complete and submit the Vendor/Participant Complaint form:

Once the Vendor/Participant Complaint form has been completed, it may be submitted by one of the following methods:

Examples of Complaints about Vendors

Complaints about vendors may include, but are not limited to:

  • Adverse treatment of a WIC participant by a Vendor’s owner, manager, employee, or other vendor representative
  • Charging WIC participants higher prices than other customers
  • Providing outdated or spoiled foods
  • Refusing to accept manufacturers’ coupons or refusing to honor store discounts or promotions that are available to non-WIC customers
  • Inadequate variety and/or quantity of WIC foods
  • Fraudulent program violations, such as: vendor trading cash, alcohol, tobacco or firearms in exchange for WIC benefit; allowing exchanges of non-approved WIC foods; providing a tangible incentive for WIC participants to shop at the store that is not offered to non-WIC participants; and refusing to provide the participant with a receipt of the WIC transaction
Examples of Complaints About Participants

Complaints about participants may include, but are not limited to:

  • Adverse treatment of a vendor’s owner, manager, employee, or other vendor representative
  • Purchase or attempted purchase of unauthorized foods with WIC benefits
  • Attempt to exchange WIC benefits for unapproved foods, cash, alcohol, tobacco or firearms

Reporting Fraud and Abuse

In order for the WIC Program to remain a success it is imperative to ensure that WIC program dollars are provided only to eligible families and that participants are receiving the nutritional foods determined beneficial for their family’s needs.

Reporting WIC Fraud and Abuse

There are several ways to report fraud and abuse.

    Messages may be left 24 hours a day, 7 days a week. Callers may remain anonymous.
Examples of Vendor Fraud and Abuse

WIC vendor fraud may include, but is not limited to:

  • Vendor trading cash, tobacco, alcohol or firearms in exchange for WIC benefits
  • Vendor removing participant’s benefits without providing WIC foods
  • Vendor altering food prices from what is posted
  • Vendor substituting non-approved foods
  • Vendor in possession of participant’s eWIC card
  • Vendor asking participant for the eWIC card PIN
Examples of Participant Fraud and Abuse

WIC participant fraud may include, but is not limited to:

  • Participant attempting to sell or trade WIC benefits
  • Participant attempting to sell items purchased with WIC benefits (including infant formula)
  • Participant providing false information in order to qualify for WIC benefits
