Anticipated Annual Award Amount: Up to $50,000, per organization, per year, up to 3 years.

Grant funding is expressly dependent upon the availability of funds to the Wisconsin Department of Health Services (DHS) from the Health Resources and Services Administration, Title V Maternal and Child Health Grant. Applicants will be awarded based on available funding, capacity, and ability to meet application criteria. This project period begins January 1, 2023 and ends December 31, 2025. Each contract will have a duration of one calendar year with the possibility of annual renewal for the duration of the project period, contingent on funding availability and performance. Should additional funding become available, award amounts may be altered.

Organizations receiving this award will be expected, at a minimum, to perform the following activities:

  • Recruit members of the identified Title V focus population(s) to be leaders and advocates for the needs of their communities in the context of Title V programs
  • Provide training and support for leadership development with members of the Title V focus population(s)
  • Create or facilitate a framework for community members to provide input and feedback to Title V-funded programs (this could take different forms, like a consultant model, a community advisory board model, or other framework for community members to provide specific input and participate in program planning and decision-making)
  • Provide appropriate compensation for the time and expertise offered by recruited leaders
  •  Continuously evaluate engagement and satisfaction of youth, caregiver, and family leaders, and provide feedback to the Title V program on improving community engagement
