How to Apply
To apply, contact the Katie Beckett Consultant
located in the region where the child lives. The Katie Beckett Consultant will
explain the program and assist the family in deciding whether or not the
Katie Beckett Program is the best Medicaid option for their child. If a parent decides
to apply, the Consultant will send an application packet. The
application is also available online as a Word fillable document. This
document can be saved to a personal computer and printed it out. Once
the application and the forms from the application packet are complete,
the parent should call the consultant to schedule a home visit to meet
the child, go over the application packet and answer questions.
The application cannot be electronically submitted for multiple reasons.
The most important reason is that the application contains very personal
and sensitive information about your child. The Department does not have
the ability to encrypt incoming e-mails. In addition, a home visit is
required with new applications. The consultant will submit the completed
application packet to the Department after meeting with your child in
