A handbook will be mailed to each family who applies for a ForwardHealth Plan.

In addition, as of April 1, 2014 the BadgerCare Plus Core and Basic Plans have ended. Information for individuals who were enrolled in the Core Plan or the Basic Plan and their health care options is available on the  Core Plan home page or the Basic Plan home page
