DHS DQA Provider Portal: MyWisconsin ID

MyWisconsin ID is the State of Wisconsin's identity solution that allows citizens the ability to securely access participating online state services and systems using a single user ID and password. Users will use their MyWisconsin ID to access the DHS DQA Provider Portal.

You already have a MyWisconsin ID if you use the Entity Background Check and Misconduct Reporting System. You will not need to register for a new MyWisconsin ID account if you already have one for the purposes of accessing those systems and can use that same log in for the DQA Portal.

If you already have a MyWisconsin ID, please navigate to the DHS DQA Provider Portal website. Click on "Sign in with My Wisconsin ID (Not a state employee)" to log in.

Create your MyWisconsin ID

  1. Visit the MyWisconsin ID webpage.
  2. Click Create Your MyWisconsin ID.
  3. Follow the instructions to create your login. The link to account creation is under Step 1 on the webpage.
    • MyWisconsin ID requires multi-factor authentication (MFA). Set up your authentication method of choice as part of your account creation. You must set up at least one method.
  4. Once your account is created, you can close the MyWisconsin ID webpages.

Once your MyWisconsin ID is created, you can now log into the DHS DQA Provider Portal website.

Signing into the portal

  1. Navigate to the DHS DQA Provider Portal website.
  2. Click on "Sign in with My Wisconsin ID (Not a state employee)" to log in with your MyWisconsin ID credentials.
  3. Create your DHS DQA Provider Portal account.

Forgot password

If you forgot your password, go to the login page, enter your email address, click Next, and then click the Forgot Password link to reset through email. If you do not receive an account recovery email when using the Forgot Password link, it means you do not have a MyWisconsin ID account associated with that email address. You need to create an account with the email address you want to use with MyWisconsin ID. If you receive the error message, “A user with this Email already exists" when creating an account, call 608-471-6667 for assistance.

Wisconsin state employees

Wisconsin state employees will not use MyWisconsin ID to access the DHS DQA Provider Portal. Wisconsin state employees will use their state employee credentials to log into the portal.
