As a health care provider and professional, you are a key partner in connecting families to WIC, and helping WIC participants make healthy choices. Wisconsin WIC serves more than 90,000 infants, children, and pregnant, breastfeeding, and postpartum people annually. We value your partnership and look forward to working with you! Help your patients receive WIC benefits
You play a vital role in helping Wisconsin families find the WIC services and support they need. Here are ways you can help your patients:
- Tell your patients about the WIC program. Display WIC outreach brochures in your office and include in new patient materials.
- Refer your patients to the Wisconsin WIC program. Use section 1 of the Wisconsin WIC Referral/Request for Medical Formula/Food (infant/child, F-44024d (Word) or pregnant/breastfeeding/postpartum, F-44024a (Word)).
- Send height, weight, and hemoglobin measurements to their WIC office. This allows WIC participants to receive remote services. Use section 1 of the Wisconsin WIC Referral/Request for Medical Formula/Food (infant/child, F-44024d (Word) or pregnant/breastfeeding/postpartum, F-44024a (Word)).

Request a medical formula or food
WIC requires documentation of a qualifying medical condition(s) to provide medical formula and food.
- Review the Wisconsin WIC Formulary, P-40077 (PDF)
- Complete the Wisconsin WIC Request for Medical Formula/Food for an infant or child, or for a pregnant, breastfeeding, or postpartum person.
- Send the form to your patient’s local WIC office.
