Help your patients receive WIC benefits
As a health care provider and professional, you are a key partner in connecting families to WIC and helping WIC participants make healthy choices.
Refer patients to WIC or request a medical formula or food
- Complete section 1 of the Wisconsin WIC Request for Medical Formula/Food for an infant or child, or for a pregnant, breastfeeding, or postpartum patient. Send height, weight, and hemoglobin measurements, which allows participants to receive remote services. Only section 1 is required if no medial food or formula is needed.
- If a medical food or formula is needed, review the Wisconsin WIC Formulary, P-40077 (PDF) and complete the entire Wisconsin WIC Request for Medical Formula/Food for an infant or child, or for a pregnant, breastfeeding, or postpartum person. WIC requires documentation of a qualifying medical condition(s) to provide medical formula and food.
- Send the form to your patient’s WIC office.
Tell your patients about the WIC program.
Display WIC outreach brochures in your office and include in new patient materials. Contact your local WIC office to order.
Encourage colleagues and new staff to refer patients to WIC.
