2011 and 2012 Consumer Information Reports for Nursing Homes
and Facilities for People with Developmental Disabilities
The Consumer Information Report is produced annually by the
Division of
Quality Assurance for each Wisconsin nursing home and facility serving
persons with developmental disabilities (FDD - also known as ICF/IID).
The report provides information to aid residents or prospective
residents in assessing the quality of care provided by a facility.
Information includes facility specific federal violations and staffing
information.
Please note that there are a few facilities listed that do
not have reports. They are either state licensed only (i.e. not
accepting patients funded by Medicare or Medicaid) or else were
established too recently to generate reports. Also note that
facilities that ceased operations in the reporting period do not appear
on this report.
