Children’s Long-Term Support Waiver Program Individual Service Plan Information Sharing

Federal regulations require Children’s Long-Term Support Waiver Program individual service plans be signed and distributed to providers who are responsible for the plans’ implementation. The federal Centers for Medicare & Medicaid Services (CMS) requested changes to the Wisconsin Department of Health Services’ initial approach to fulfilling the requirement. As a result, DMS Numbered Memo 2018-05 has been rescinded and replaced with this memo.
