In Wisconsin, providers performing clinical laboratory testing must get a federal certificate to conduct certain testing.

Clinical Laboratory Improvement Amendments (CLIA) certification is for laboratories. Federal CLIA certification is required for those who analyze material from the human body to diagnose, prevent, or treat a disease, or assess human health.

Two years

Fees vary based on the certificate type and test volume.

To apply, download and complete the federal CMS-116 CLIA Application for Certification (PDF). The CMS (Centers for Medicare & Medicaid Services) has details about How to Apply for a CLIA Certificate, Including International Laboratories.
