Background check applications for all users

To prepare for the transition to the new DHS DQA Provider Portal, the Division of Quality Assurance (DQA) will not accept any new background check applications for entity operators, non-client residents, or other required individuals for new or renewing entity license/certification applications, or for rehabilitation review applicants as of November 22, 2025.

DQA will resume accepting background check applications on December 15, 2025, through the DHS DQA Provider Portal. To prepare for this transition and for more information, visit our webpage. Caregiver background checks completed by entities for employees and contractors are not affected by this transition.

In Wisconsin, pain clinics must be licensed.

Description

A pain clinic license is for privately owned facilities that have providers or services focused on treating pain. The clinic can either:

  • Have a majority of its health care providers, within the scope of their license, devote most of their time to treating pain conditions. This includes the practice of pain medicine or interventional pain medicine.
  • Advertise and reserve time to provide pain medicine or interventional pain medicine services. The clinic must have one or more worker who prescribes any of the following as chronic therapy for pain:
    • Opioids or opiates
    • Benzodiazepines
    • Carisoprodol

The Division of Quality Assurance (DQA) sends renewal information at least 60 days before your license expires. You must return materials 30 days prior to expiration.
